I charge £25pm for these which include one blog post / giveaway / mention per month at your request. Buttons are 125px x 125px and are set within a gadget that alternates the buttons to keep readers' interest (readers are known to become immune to static buttons on the sidebar of a blog).
I am also happy to discuss sponsorship paid in goods - eg a blogpost / giveaway in exchange for products / fabrics / wadding / notions / tools / BUT ONLY if the products / fabrics books are those which I would anyway use - eg those used for the kinds of quilting I do.
